We are currently seeking a Social Service Assistant who is compassionate, dependable, and dedicated to enhancing the emotional and social well-being of our residents. If you’re ready to make a meaningful impact as part of a collaborative care team, we’d love to meet you.
What You’ll Do:
Support the emotional, social, and psychological needs of residents
Help develop and maintain individualized care plans
Participate in resident admissions, discharge planning, and transitions
Attend and contribute to interdisciplinary care plan meetings
Build trust with residents and families to help ease transitions and support overall well-being
What We’re Looking For:
Bachelor’s degree in Sociology or related field preferred
Previous experience in long-term care or skilled nursing preferred
Compassionate communicator with strong interpersonal skills
Organized, reliable, and team-oriented
