JOB DESCRIPTION

SOCIAL WORK ASSISTANT

Responsible for fulfilling the need for social service evaluations by B.S.W.

QUALIFICATIONS

  • A qualified Social Work Assistant is a person who has a baccalaureate degree in social work, psychology, sociology, or another related field, with one year social work experience in a health care setting preferred .
  • Must be capable of performing job duties.

KEY RESPONSIBILITIES

  1. Completes initial assessment visit to patient within 5-7 days of receipt of referral from nursing staff or physician and develops a care plan.

 

  1. Completes all required paper work associated with visits on a daily basis.

 

  1. Writes supplemental orders as needed and within 24 hours after receipt of verbal orders from the physician.

 

  1. Submits all documentation associated with daily visits within 24 hours of visit date.

 

  1. Prepares daily route and time sheet; submits within 24 hours of visit date.

 

  1. Maintains all records and information relevant to the patient for purposes of their service provided in coordination with the general policies of the agency.

 

  1. Assists the physician and other members of the health team in understanding the significant social and emotional factors related to the patient’s health problems.

 

  1. Assesses the social and emotional factors in order to estimate the patient’s capacity and potential to cope with problems of daily living.

 

  1. Helps the patient and his family to understand, accept and follow medical recommendations. Provides services planned to restore the patient to optimum social and health adjustments within his capacity.

 

  1. Assists patient and their families with personal and environmental difficulties which predispose toward illness or interfere with obtaining maximum benefits from medical care.

 

  1. Utilizes resources, such as family and community agencies to assist patient in resuming life in the community or learning to live with his disability.

 

  1. Participates with agency staff to determine source of payment for services to be rendered.

 

  1. Participates in conferences as needed or required to coordinate the care of an individual patient.

 

  1. Conducts inservices as needed to agency personnel.

 

  1. Attends in-services, seminars, or other meetings as assigned by the immediate supervisor or other management.

 

  1. Maintains confidentiality of information related to business practices, business activities, and personnel.

 

  1. Maintains a professional attitude when working with staff, fostering the team philosophy and team environment for all personnel.

 

  1. Does not allow personal affairs to interfere with scheduled work time.

 

  1. Maintains a professional appearance at all times.

 

  1. Provides proper notification and/or advanced notice of absence or tardiness without abuse.

 

  1. Participates willingly with special projects and/or overtime work when requested.

 

  1. Follows all agency policies and procedures.
